Griffin Health and the Naugatuck Valley Health District are looking for input from Lower Naugatuck Valley residents to help shape the future of community-based support services.
Residents of Ansonia, Beacon Falls, Derby, Naugatuck, Seymour, Shelton and Oxford are encouraged to take a brief, online survey to identify community needs and priorities for the Valley’s Community Health Needs Assessment. A link to the survey can be found at nvhd.org/community-survey.
“Griffin Health and the Naugatuck Valley Health District are working with people throughout the Valley to develop our next Community Health Improvement Plan,” said Monica Oris, Director of Population Health at Griffin Health. “As part of this work, it is essential to hear from community members aged 18 or older about what matters most to them right now.”
Data from the Community Health Needs Assessment, which is written and released every three years, is used to develop the Valley’s Community Health Improvement Plan. This plan outlines interventions, programs, and services to help address community priorities for the next three years.
The anonymous and confidential survey ends June 15.
