My Exercise Goals
Check out the CDC’s webpage on “Healthy Weight, Nutrition, and Physical Activity” to help you make your goals. If you would like recommendations on creating an exercise plan, please speak with your provider. If you have been diagnosed with a serious health condition, please speak with your provider before beginning any exercise activity.
- Example – I will walk for 30 minutes at least 5 times a week or I will do 30 minutes of strength training at least two times a week.

- Example – I will drink 8 glasses of water a day or I will have a snack of fruits and/or vegetables once a day.
My Overall Health Goals
Set specific goals and when you hope to achieve them.
For example, “I will document what I eat and drink to share with my provider for recommendations” or “I will maintain my weight by the end of February and will try to have lost weight by the end of April.”
