We are happy to help you access your medical records, whether you received care at Griffin Hospital or from a Griffin Faculty Physicians (GFP) medical provider. Below are your options to access your patient records:
Access Records by Submitting Requests In Person
For Griffin Hospital records, you can request records at our Medical Records Department located on the second floor of the hospital. Hours of operation are Monday – Friday 7:00 am – 6:00 pm.
For Griffin Faculty Practice Records, you may request your records at any of our physician office locations during office business hours
Submit Records Requests Through our Online Request Tool
We are happy to announce that the online request option is a one stop option to obtain both Griffin Hospital and Griffin Faculty Practice Records. Follow the prompts to select Griffin Hospital Records, Griffin Faculty Practice Records or both Hospital and Griffin Faculty Physician Offices.
We now have a fully online records request tool that ensures security through identity verification, allowing you to submit requests with ease on a computer, tablet or smartphone. Enjoy the convenience of this service at no cost. Click here to request your records online and follow the instructions carefully. This option is particularly helpful for the following individuals to request patient records:
· Patients requesting specific documents of their own records
· Parents of minor patients requesting records
· Caregivers acting on behalf of a patient (i.e. Power of Attorney)
The Online tool verifies your identity by asking for a photo of your driver’s license, which can be taken via webcam or smartphone. There is no additional charge to use this service.

Submit Records Requests Through My Griffin Health Patient Portal
To obtain online access to your medical records, you can set up access credentials for your Patient Portal. Please note that the hospital and Griffin Faculty Practice each have their own secure portals allowing you access to your patient information anywhere you have internet access. The patient portal option allows you to bring your important medical information with you wherever you go and build a better understanding of your health.
