T2 Lifestyle Change is a diabetes prevention program that helps individuals lose weight and improve their fitness level to prevent or manage type 2 diabetes.

T2 Lifestyle Change is a Diabetes Prevention Program is evidence-based and sponsored by the Centers for Disease Control and Prevention. Topics covered during this program include activity tracking, healthy eating/cooking, stress management, heart health, and goal setting. The objective of this program is for the participants to lose at least 5%-7% of their starting weight, get at least 150 minutes of physical activity each week at a moderate pace or more, work to reach their goal weight and lose more if they choose. The Program is comprised of one-hour weekly educational sessions for 6 months followed by 1-2x per month for another 6 months.
