Take Control of Your Diabetes
Griffin Hospital empowers individuals diagnosed with diabetes to take control of the health condition and get control of their lives again with a free six-week workshop offered throughout the year called The Live Well Diabetes Self-Management Workshop.
The Live Well Diabetes Self-Management Workshop focuses on several topics for those with diabetes, including:
- Meal planning, carbohydrate counting & reading nutrition labels
- Physical activity
- Blood sugar guidelines
- Sick day guidelines
- Managing medications
- Foot care
- Managing stress and difficult emotions
- Working with your health care team
- Action plans and problem solving
This program is sponsored by the Connecticut State Department on Aging, Connecticut Department of Public Health and Griffin Hospital Community Outreach and Valley Parish Nurse Program.
When
Workshops are held through group conference calls over the phone or via Zoom video conferences throughout the year. Conference calls are being scheduled for daytime and Zoom programs for afternoons, evenings, and Saturday mornings.
